Billing Information

Mentor Surgery BillingIf you have provided complete and accurate insurance information to your surgeon’s office, a member of our business office staff will contact your insurance company prior to surgery to verify your medical benefits. We will attempt to determine any information regarding co-payment, co-insurance, and/or deductible amounts that will be your responsibility to pay on or before your surgery.

Payment for all elective cosmetic surgeries is required in full on/or prior to surgery to avoid cancellation. Any patient with no insurance or whose insurance has denied coverage for the scheduled procedure must contact the business office prior to surgery to discuss financial arrangements.

For your convenience, The Mentor Surgery Center, LTD. accepts the following forms of payment: Cash, Check, Visa, Master Card, and Discover.

You will receive separate bills from the following providers of service, and any questions regarding these bills must be directed to their respective billing offices:

      • Your physician or surgeon
      • Anesthesia – if you received general anesthesia or it was necessary for an anesthesiologist to be available for your procedure, you will receive a bill from their office. 440-350-0832
      • Laboratory tests – any preoperative tests ordered prior to surgery not performed at The Mentor Surgery Center, LTD.
      • Radiology – any preoperative x-rays or imaging ordered prior to surgery or performed on the day of surgery.
      • Pathology – this will be for the examination of any tissues or specimens collected during surgery.

The business office staff at The Mentor Surgery Center, LTD. will be happy to answer any questions you may have regarding insurance coverage or billing procedures.
